Services: Assistance Planning for New Patients

During New Mexico's deinstitutionalization process the Continuum of Care Project helped assure that all individuals had an identified primary care physician prior to their move into the community. We also contacted each identified physician regarding support available through this project and assured that historical medical records were transferred.

Now that both state institutions have closed, we can assist physicians who serve this population by obtaining historical medical records, facilitating problem solving with the individuals' interdisciplinary teams and/or providing consultation.

We also intend to develop a "bulletin board" capacity with this web page which will allow anyone to post questions and receive perspectives from both Continuum staff and other web site users.
